Colic vs Normal Crying: How to Tell the Difference (0–4 Months)

Colic follows the rule of threes and peaks at 6 weeks. Here is how colic crying differs from normal newborn crying, what actually helps in Indian homes, and the red flags that are neither.

"Is this colic, or is my baby just a crier?" It is the question every parent asks around week 5. The difference matters, because colic needs a survival plan while other causes need a fix.

The rule of threes

Colic is crying that is:

  • More than 3 hours a day
  • More than 3 days a week
  • For more than 3 weeks
  • In a healthy, well-growing baby

Anything less than that is normal newborn crying, even when it feels enormous.

The timeline that helps most

Colic starts around 2–3 weeks, peaks at 6 weeks, and is gone by 3–4 months in the vast majority of babies. Knowing there is a peak — and that it ends — is genuinely the most useful thing you can hold onto.

What actually helps

  • Feed positioning: upright feeding and paced bottle feeding reduce air swallowing
  • Thorough burping: 5–10 minutes upright after every feed, plus mid-feed
  • Tummy work: bicycle legs, gentle clockwise tummy massage, warm ajwain potli compress
  • Tummy-down forearm hold (the "colic hold")
  • White noise and motion: fan hum, a wrap, a slow walk
  • Cut the evening chaos: dim lights, fewer visitors, one calm caregiver
  • Probiotic drops (L. reuteri): some evidence for breastfed babies — ask your paediatrician first
  • For breastfeeding mothers: trial removing dairy for 2 weeks only if allergy is suspected, and only with medical guidance

Gripe water and home remedies are widely used in India; many are harmless, but skip honey (botulism risk under 1 year), skip castor oil, and never put hing inside the navel or mouth.

What is neither colic nor normal — see a doctor

  • Fever ≥100.4°F under 3 months
  • Poor weight gain, or crossing percentile lines downwards
  • Vomiting that is green or projectile, or blood or mucus in stool
  • Eczema plus crying plus loose stools (possible cow's milk protein allergy)
  • A weak, shrill or moaning cry, floppiness, or a bulging soft spot
  • Crying that suddenly changes character and stays inconsolable for 3+ hours

Look after the parents

Colic has one of the strongest links to parental burnout and postnatal depression of anything in the newborn period. Split the evening witching hour into shifts.
